56 yr old male with dengue fever and AKI

A 56 year old male came to casualty with h/o fever since 10 days , on and off , not associated with chills , insidious in onset, no diurnal variation.
No h/o cough, cold, headache.
H/o decreased urine output since 2-3 days.
No h/o burning micturition
Loss of appetite present.
No h/o pedal edema , hematuria, frothy urine , SOB.
Not a k/c/o DM, HTN, asthma, TB , epilepsy.

Personal History :
Appetite - decreased
Diet - mixed
Bowel and bladder movements - regular
Sleep - adequate

On examination : 
Patient was c/c/c.
CVS : S1 S2 +
RS : NVBS heard
CNS : NAD

Vitals :
Temperature - 98.6 F
Pulse rate - 92 bpm
BP - 110/70 mm Hg
RR - 16/min
SpO2 - 98% at room air
GRBS - 128 mg/dl

Provisional diagnosis : 
Dengue fever with NS1 Antigen positive with renal / pre renal AKI 

Treatment : 
IVF NS, RL 100 ml/hr 
INJ pantop 40 mg/IV / stat 
INJ neomol 100 mg /iv/ if temp > 101 F 
Tab PCM 500 mg /po/od 
INJ ZOFER 4 MG / iv / sos 
INJ lasix 40 mg iv / bd , if sbp > 110 mmHg
